This editable field trip permission slip for teachers includes editable form, checklists to keep track of who has turned in their permission slips, and reflection pages to write about field trips! Use for your zoo field trip, visit to the zoo, or wherever you go–it’s versatile enough that it can be used all year for any grade! Edit any of the text on the permission form (perfect for making multilingual), including the name of the field trip, the date, time, the location, and any special requirements or instructions for the students. The field trip reflection writing form comes in 3 options (drawing, drawing with primary writing lines and drawing with intermediate writing lines) includes editable text areas as well so you can customize the prompt.
This permission slip is designed to be user-friendly and versatile for any grade level. In addition, this editable permission slip includes checklists with up to 30 names that allows you to keep track of which students have turned in their permission slips (with options of 1, 2 and 3 columns to track things like volunteers, drivers, school lunch, etc.–headings are also editable!).
